- Inscribed within a moulded tabella ansata (die, w: 0.44 x h: 0.36).
- Letters:
- Irregular capitals with some Rustic forms: l. 1, 0.04; ll. 2-6, 0.03.
- Date:
- Second to third century A.D. (lettering)
- Findspot:
- Lepcis Magna: Tomb near the ruins of Lepcis. Seen by Clermont-Ganneau (CRAcad. 1903, 340) < près d'un sépulchre récemment fouillé, un peu avant d'arriver aux ruines de Leptis>, presumably between Lepcis and Homs.

Translation:
To Calpurnia Bargydden and Calpurnius Cerealis and Calpurnius Candidus; Calpurnia Licinia Sadith had (this) made for her parents and brother, from dutiful respect.
