- Inscribed on one face within a moulded panel (die, w: 0.38 x h: 0.62); the first word is cut above the die, between formal sprays, and the last line below it. The base was formerly inscribed on a second face also, from which the text has since been completely erased.

Translation:
(In honour) of Petasius. To a man of outstanding integrity, incomparable prudence and total goodness, Flavius Petasius, excellent man (of equestrian status), former governor of the province of Tripolitania; the city council of the most splendid city of Lepcis Magna, in accordance with its own decree and the votes of the people, having in mind the benefits they enjoyed from his presence, set up a statue to a most worthy patron in the new forum Severianum with a view to their enduring memory.
